The policy includes an Accelerated Death Benefit for terminal illness as a rider policy.

Coverage for your spouse and dependent children is available subject to their meeting certain eligibility requirements.

Voluntary/Supplemental Group Term Life offers features that include:

• Conditional Guaranteed Issue

• No Medical Exams-(depending on amount of coverage) if you have not been hospitalized or missed more than 5 consecutive days from work due to accident or sickness in the past six months, you are approved.

• Accelerated Death Benefit for Terminal Illness-The Accelerated Death Benefit is a Rider Policy.

• Member Coverage:

  • Active members-coverage is available in $25,000 increments up to $300,000 or five times the applicant's annual base salary (excluding bonuses, commissions, overtime and all other remuneration other than base pay), whichever is lower.

  • Retired Members-coverage is available in $25,000 increments up to $100,000.

   

• Members who leave the Group can continue coverage on a direct bill basis with easy monthly payments via credit card or automatic checking account deduction.

• Optional Spouse and Children’s Coverage Available  

• Applications are processed over the phone

• Easy monthly payments via credit card or pre-authorized bank draft
